目的:了解老年人医疗资源消耗分布及费用水平,总结医疗资源消耗特点并提出相应政策建议。方法:以上海市常住人口为统计口径,对医疗机构医疗服务利用和费用数据开展描述性分析和比较分析。结果:不到20%的老年人发生的门急诊人次、出院人数分别约占全市总量的60%、45%,门急诊费用和住院费用分别占全市总量的60%、50%以上;与其他人群相比,老年人医疗费用更多流向基层,病种结构主要集中在循环系统、恶性肿瘤、呼吸系统疾病;人均医疗费用随年龄增加整体呈上升趋势,老年人口的住院费用个人负担比例较低。结论:老年人利用了更多的医疗服务,特别是基层医疗服务,且其医疗支出以药品为主,部分老年人的就医负担较重,需进一步满足康复护理需求。
摘要(英文):
Objective: To understand the distribution and cost of medical resources in the elderly, summarize the characteristics of medical resources consumption and propose corresponding policy recommendations. Methods: Based on the statistical data of resident population in Shanghai, descriptive and comparative analysis of medical service utilization and cost due to medical treatment were made. Results: 19.5% of the elderly had outpatient and emergency visits and discharges, accounted for approximately 60% and 45% of the total. Expenses of outpatient and emergency visits and hospitalization expenses accounted for more than 60% and 50% of the total; compared with other population, the medical expenses of the elderly tended to flew to the grassroots. The disease structure mainly concentrated in the circulatory system, malignant tumors and respiratory diseases. The average per capita medical expenses increased with the age, and the out-of-poeket(OOP) proportion of hospitalization expenses in the elderly was relatively low. Conclusion: The elderly utilized more medical services, especially primary health services. The medical expenditures were mainly medicine. Some elderly people had a heavy medical burden and their needs of rehabilitation and nursing should be further met.
